Welcome to Hoops N Dreams!

Thank you for choosing Hoops N Dreams.

We are committed to providing a positive, competitive, and supportive basketball environment where players can develop their skills, build confidence, learn teamwork, and grow both on and off the court.

PLEASE READ BEFORE COMPLETING REGISTRATION.

This registration form is required for all players participating in Hoops N Dreams.

Parents/guardians are responsible for providing complete and accurate information for their player, including current contact information and emergency contacts.

Registration does not guarantee a specific team, coach, practice day/time, playing position, playing time, tournament, or event.

Team placement and scheduling are determined by Hoops N Dreams based on program needs, player evaluations, availability, and other appropriate factors.

Players and parents/guardians are expected to follow all Hoops N Dreams Team Rules, Code of Conduct, health & safety requirements, facility rules, and program policies.

PAYMENT & REGISTRATION

Registration is not considered complete until all required registration information and applicable payments have been received.

Parents/guardians are responsible for paying applicable registration fees and monthly team dues according to the current Hoops N Dreams payment schedule.

Policies, Procedures & Waivers

I acknowledge that Skill Evaluations are compulsory. Player requests will not be taken into account if your player does not participate in the skill evaluations.

I understand the MANDATORY Skill Evaluations that my player must attend will be held

I have read and agree to Hoops N Dream's Code of Conduct. A copy of our Code of Conduct can be found on our website

I am aware that my player's team may travel outside of their local area on Saturdays during the season to play against teams within other Hoops N Dreams chapter

I understand that practice locations may be held at all schools/gyms within the area school district and will not be determined by where my player resides or goes to school. I am aware that there may be traveling involved to get my player to practice

I understand that player/team/coach requests are never guaranteed

I understand that the division I am registering for is Co-Ed

I understand that when registering my daughter in a co-ed division there is no guarantee how many other girls will be placed on her team

I UNDERSTAND THAT IF HOOPS N DREAMS IS FORCED TO CLOSE DUE TO COVID OR A GLOBAL PANDEMIC OUR FEES PAID ARE NON-REFUNDABLE

Uniforms must be selected separately, with the option to purchase 1-3 sets, as required.

Please note that uniforms are pre-ordered, and if you do not select and pay for the needed uniform now, availability cannot be guaranteed at a later date.

Agreement, Waiver, & Release:

REGISTRATION AGREEMENT, PARENT/GUARDIAN CONSENT & ACKNOWLEDGMENT

I am the parent or legal guardian of the player identified on this registration form, and I give permission for my child to participate in Hoops N Dreams basketball practices, games, tournaments, training sessions, team activities, and events.

I understand that participation in basketball and other athletic activities involves inherent risks, including falls, collisions, physical contact, and other injuries.

I voluntarily permit my child to participate and acknowledge these inherent risks.

To the fullest extent permitted by California law, I acknowledge and accept the ordinary inherent risks associated with my child's participation.

Nothing in this agreement is intended to waive or limit any rights or liabilities that cannot legally be waived or limited under California law.

PLAYER & PARENT RESPONSIBILITIES

I agree that my child and I will follow all applicable Hoops N Dreams:

• Code of Conduct
• Health & Safety Protocols
• Coach instructions
• Tournament requirements
• Gym and facility rules
• Registration and payment requirements
• Fundraising and sponsorship rules

I understand that serious or repeated violations of these requirements may result in disciplinary action, including warnings, suspension, removal from an activity, or removal from the Hoops N Dreams program when appropriate.

I certify that the player's age, grade level, emergency information, and other information provided during registration are accurate.

POLICIES & COMMUNICATION

I understand that I am responsible for reviewing official Hoops N Dreams information provided through the website, registration system, email, text message, handouts, schedules, and other official communications.

I acknowledge that I have been provided access to and am responsible for reviewing the current Hoops N Dreams policies, including:

• Health & Safety Protocols

NO-REFUND POLICY

I understand that Hoops N Dreams maintains a no-refund policy as described in the organization's current written policy.

I understand that a player's decision to quit, stop attending, become inactive, or leave the program does not automatically create a right to a refund of registration fees, team fees, monthly dues, or other payments that are designated as non-refundable under the applicable policy.

FUNDRAISING, DONATIONS & SPONSORSHIPS

I understand that fundraising proceeds, donations, and sponsorship contributions collected through Hoops N Dreams are intended to support legitimate team and program-related expenses.

I understand that these funds are not personal funds belonging to my child or me and are not maintained as personal cash accounts.

I understand that fundraising proceeds, donations, and sponsorship contributions cannot be redeemed for cash or paid directly to a player, parent, or guardian as provided in the applicable Hoops N Dreams policy.

If my child voluntarily leaves, stops participating, becomes inactive, or is removed from the program, I understand that unused fundraising and sponsorship proceeds remain subject to the Hoops N Dreams Fundraising & Sponsorship Policy.

EMERGENCY AUTHORIZATION

If I cannot be reached during an emergency involving my child, I authorize Hoops N Dreams personnel to contact emergency services and take reasonable steps to obtain necessary emergency care.

I understand that I remain responsible for medical expenses not otherwise covered.

PARENT/GUARDIAN ACKNOWLEDGMENT
